Field Experience and Student Teaching Placement Requests
(for CCS employees and non-employees)

Step 1: 
Complete the Student Teacher / Field Experience / Observation Request Form a semester in advance. 
This form must be completed each semester.

Step 2:
Complete the Application for Employment (non-employees only). 
Once the application has been completed, it must only be updated each semester for new placements. 

Step 3: 
Email Dr. Jaime Garrett and email Cindy Meadows  once the previous steps have been completed. 

 Note:  Include the following information attached to the email:  

• College / University placement request form with details.

• Memorandum of Agreement

CCS employees must also include the building administrators on the email.  

Step 4: 
Complete required forms sent via email for security clearance purposes.  

Step 5: 
Wait for clearance approval designation notice sent via email to the applicant and school administrator.      
Note: Approval must be granted prior to placement.

Complete all of the above listed steps and submit the linked form to the HR office and your supervisor (Principal) at least one semester in advance of the requested placement.
